This site allows you to organize and store your medical history. A medical history consists of medical conditions, procedures and tests, medications, allergies, vaccines, family history, healthcare providers, pharmacies, and emergency contacts.

You can store your medical history for your own reference or share it with a healthcare provider by providing them a printed copy or allowing them to view it online using your account number. Sharing your history with healthcare providers will allow them to improve their care.

Your information is securely stored on Amazon Cloud Services and is never shared. Our Privacy Policy describes our efforts to maintain your privacy.
